Health Form Information and Requirements

Magnus Health

Beginning June 1st, 2016, Saint Anselm’s Abbey School will be shifting all student health forms and medical records to an online database called Magnus Health.  

Magnus Health is a web-based system that will give parents continuous access to their child’s medical records as well as the ability to make updates when needed. Magnus Health will also provide us with a secure and more efficient way to collect and manage our students’ health forms and medical records and to communicate with parents.  Please note that starting June 1st you will retrieve from and submit all completed health forms directly to Magnus Health via their web-based system. Any health forms submitted directly to the School will not be accepted and will be returned to sender.  We will work with you every step of the way to make sure this transition is as smooth as possible. 

You will receive an email before June 1st with your username and temporary password (which you must then personalize) to access your student’s Magnus Health account.  Starting June 1st, you will have access to your Magnus Health account via the button titled “Magnus Health Portal,” both on the School’s home page as well as on the "Health Forms" section of the "Parents" web page.  After login in, watching the Getting Started Video, and setting up your student’s account you will have to complete all of the requirements in your “to do” list. The majority of the requirements are health forms that must be printed, completed by you and your doctor, and uploaded back to your student’s account to be submitted for approval. The forms can be uploaded via scanner or by taking a picture of the forms with your smart phone (in order to use the picture option you must be logged into your Magnus Health account via your smart phone’s internet browser). The forms can also be directly mailed or faxed to Magnus Health. Please make sure to include the barcoded coversheet that is generated specifically for each student, for each requirement when mailing or faxing forms as coversheets cannot be shared between siblings or classmates.  There are a few forms that can be signed electronically thus would not need to be printed.  

We ask that you complete the health information and forms required within your Magnus Health account for each student attending Saint Anselm’s Abbey School no later than August 12th 2016 (July 29th for students who plan to play varsity soccer)Please note that no students will be allowed to attend classes (or participate in varsity soccer pre-season) until all required health forms have been completed and approved.

Health Forms

All required health forms can be found and printed in your student’s Magnus Health account “to do” list.

Please note that except for vaccination records which can be attached  to the DC Universal Health Certificate no other forms can be substituted. Any substituted form will not be accepted nor approved as a fulfilled requirement. 

Forms Required for All Students
  • Permission to Treat and to Administer Over-The-Counter Medications in School Form:

This form gives the school permission to administer basic first aid and medical care to your student as well as permission to administer specific over-the-counter medications while in school. The form also grants your student permission to participate in PE and Intramural classes to the extent permitted by your physician by way of a required yearly physical exam. This form is required for all students and must be completed every school year and signed by a parent/guardian and physician.

  • DC Universal Health Certificate and Vaccination Records Form:

This form documents immunizations records, tuberculosis assessment, lead screening and a physical exam. The form MUST be fully completed in order to be approved. Please note that the physical examination portion of this form is valid for one full year from the date of administration and must be dated after June 1st of the same year in order to be valid for the entire upcoming school year. Students with a physical expiring in the middle of the school year will be required to obtain a new physical prior to the previous one expiring in order to avoid any gaps in participation. Also note that it is okay to attach your son's vaccination records form from your doctor's office to this form (this is the only instance in which a different form other than the ones provided by the school will be accepted). This form is required for all students and must be completed every school year and signed by a parent/guardian and physician. 

*When attaching vaccination records from your doctor's office make sure that both the records  as well as the DC Universal Health Certificate form get uploaded into your Magnus Health account together as one document.

  •     DC Oral Health Assessment Form: 

This form is to be completed by your dental provider. A parent/guardian must also sign and date Part 2 of this form. This form is required for all students and must be completed every school year and signed by a parent/guardian and physician. 

  • Parent/Athlete Concussion Information Sheet:  

This form is required for all students and must be signed once when first entering the School. Please note that due to our transition to Magnus Health we are asking all families (including current families that have already signed a hard copy version of this form) to sign the digital version this year.  This is an educational form on the topic of Concussions. The form must be read by the student and a parent/guardian and signed. 

  • Concussion Management Program Consent for Testing Form:  

This form is a required for all students that wish to participate in our Concussion Management Baseline Testing Program. Please review and sign the Consent Form if you would like to authorize your student to complete the test. Please note that due to our transition to Magnus Health we are asking all families (including current families that have already signed a hard copy version of this form) to sign the digital version this year.

Forms Required for Some Students:  

  • Authorization for Prescription Medication Administration Form (If applicable):

This form is required only for students that take a prescription medication during school hours. This form gives the School permission to administer a medication prescribed by a physician and must be completed every school year and signed by a parent/guardian and physician. 

  • Authorization for Medical Procedure/Treatment Form (If applicable):  

This form is required only for students that might need a medical procedure/treatment (insulin, epi-pens, inhalers, etc.) during school hours. This form gives the School permission to assist a student with a specific medical procedure/treatment ordered by a physician and must be completed every school year and signed by a parent/guardian and physician. 

  • Asthma Action Plan Form (If applicable):  

This form is required only for students with asthma. This form gives the School permission to implement an asthma action plan prescribed by a physician and must be completed every school year and signed by a parent/guardian and physician. 

  •     Anaphylaxis Action Plan Form (If applicable): 

This form gives the School permission to implement an anaphylaxis action plan prescribed by a physician. This form is required only for students with food and other allergies that might require the use of an Epi-Pen during school hours and must be completed every school year and signed by a parent/guardian and physician. Please remember that Epi-Pens also have an expiration date of 12 months and your student will probably need a new one every time this form is renewed. 

  •   Diabetes Action Plan Form (If applicable): 

This form is required only for students with diabetes. This form gives the School permission to implement a diabetes action plan prescribed by a physician and must be completed every school year and signed by a parent/guardian and physician. 

  • Human Papilloma Virus (HPV) Annual Vaccination Opt-Out Certificate Form (If applicable):  

This form is required for all students choosing to opt-out of the HPV vaccine as well as those considering starting the vaccine in the future but had not received the first dose yet.  HPV immunization compliance is a requirement for school attendance in the District of Columbia. Students can be compliant with the requirement by either receiving all three doses in the HPV vaccination series on time, or by completing and submitting an HPV Opt-out form. Note that the HPV Opt-out Certificate is only valid for one (1) school year so it must be completed and submitted every year by students choosing to opt-out.
